چکیده

Theresa M. Wadas is a coordinator for the adult acute and continuing care nurse practitioner program at the University of Alabama at Birmingham, Birmingham, Ala. Heart failure is a clinical syndrome that affects 5 million persons in the United States and results in substantial morbidity and mortality. Despite advances in pharmacological management, 50% of patients with chronic heart failure will die within 5 years of the diagnosis. The progression of left ventricular dysfunction and severe symptomatic decompensation often requiring hospitalization remain essential features of the heart failure syndrome. Heart failure is responsible for 11 million visits to healthcare providers’ offices, causes or contributes to more than 3.5 million hospitalizations annually, and remains the leading cause of hospitalization for those 65 years or older. The estimated economic burden of heart failure is $20 billion to $40 billion per year. This amount includes the cost of care delivered by healthcare providers, expenses related to hospital and nursing home services, and the cost of medications, home healthcare, and other medical durables. Managing patients with heart failure requires an accurate clinical assessment of the patients’ hemodynamic status. Subtle changes in volume status or intracardiac filling pressures may affect functional status, hemodynamic status, ventricular performance, and survival. In particular, elevated filling pressures (ie, increased jugular venous pressure, central venous pressure, right and left atrial and ventricular pressures, and pulmonary artery pressure) may lead to neurohormonal activation, which acts as a compensatory mechanism to support a failing heart. Without appropriate interventions, persistent neurohormonal activation leads to worsening cardiac function and deleterious outcomes of advanced heart failure. In order to prevent progression of heart failure, patients require frequent physical examination and judicious monitoring of their hemodynamic status to optimize therapy. Unfortunately, patients with comThe Implantable Hemodynamic Monitoring System CoverArticle

Evaluation of Cardiac Function in Patients with Brain Death using Advanced Hemodynamic Monitoring

Background: Studies on hemodynamic changes in brain death are in vitro and in animal studies. And very few studies have been done on the hemodynamic changes of brain death. The aim of this study was to use advanced hemodynamic monitoring with echocardiography for evaluation of donated heart and to evaluate the moment by moment brain death patients with advanced hemodynamic monitoring tools and ...

Implantable hemodynamic monitoring (the Chronicle IHM system): remote telemonitoring for patients with heart failure.

(1) Remote monitoring for ambulatory heart failure patients uses an implantable device to record hemodynamic data and transmit it to a central server for continuous assessment. (2) Preliminary evidence from observational studies suggests a potential for reducing hospitalizations with the use of right ventricle implantable hemodynamic monitoring (IHM). However, although a multicentre, randomized...

Neural Monitoring With CMOS Image Sensors

Implantable image sensors have several biomedical applications due to their miniature size, light weight, and low power consumption achieved through sub-micron standard CMOS (Complementary Metal Oxide Semiconductor) technologies. The main applications are in specific cell labeling, neural activity detection, and biomedical imaging. In this paper the recent research studies on implantable CMOS i...

Hemodynamic monitoring in heart failure and pulmonary hypertension: From analog tracings to the digital age

Hemodynamic monitoring has long formed the cornerstone of heart failure (HF) and pulmonary hypertension diagnosis and management. We review the long history of invasive hemodynamic monitors initially using pulmonary artery (PA) pressure catheters in the hospital setting, to evaluating the utility of a number of implantable devices that can allow for ambulatory determination of intracardiac pres...

A System for Continuous Estimating and Monitoring Cardiac Output via Arterial Waveform Analysis

Background: Cardiac output (CO) is the total volume of blood pumped by the heart per minute and is a function of heart rate and stroke volume. CO is one of the most important parameters for monitoring cardiac function, estimating global oxygen delivery and understanding the causes of high blood pressure. Hence, measuring CO has always been a matter of interest to researchers and clinicians. Sev...
